Goals -
  • Engage the Community in developing and strengthening an integrative, whole health system of care, based on the Integrative Health Path, which provides mental health services to all regardless of age, gender, ability to pay, and disorder, within a network of key health and human service providers in Sonoma County.
  • Map out Sonoma County's "Safety Net" for the purpose of identifying the issues and strategies available and needed for community members and service providers to effectively interface and collaborate to Create a Safety Net.
Past Accomplishments -
  • Identify key service providers to engage
  • Determined barriers to creating a seamless continuum of care model
Current Projects -
  • Building relationships throughout Sonoma County to Create a Safety Net
  • Mapping the Safety Net
  • Developing a series of forums (in the 2008-09 fiscal year) and community fairs through which to engage and educate the community
Future Projects -
  • Coonvene and facilitate dialogues in and between the service provider and general public communities regarding Creating a Safety Net
  • Develop clear parameters for partnership between agencies, in particular between agencies that refer and receive referrals from each other, that will ease consumers and family member's transition as they progress in the system
  • Build a model "No Wrong Door" continuum of care by identifying and addressing areas in which members can build capacity
  • Overcoming barriers to creating a seamless system of care
